Minimal Change Disease is more common in children and teen-agers, which comprises about 90% of the primary Nephrotic Syndrome under 5 years old. The condition is called minimal change disease because children with this form of nephrotic syndrome have normal or nearly normal biopsies.
The onset of the disease is always acute, and edema is the most initial manifestations. 50% of the patients have some early infections, allergy etc. before the disease.
In the other 20 percent of children with nephrotic syndrome the kidney biopsy reveals scarring or deposits in the blood filtering units of the kidney. The two most common diseases that cause this damage are the almost unpronounceable focal segmental glomerulosclerosis (FSGS) and membranoproliferative glomerulonephritis (MPGN).
Childhood nephrotic syndrome is characterized by high levels of protein in the urine low levels of protein in the blood and swelling caused by a buildup of salt and water. The first sign is often swelling around the eyes particularly in the morning. Later the swelling may extend to the abdomen and the ankles and feet.
The common treatment for Childhood nephrotic syndrome (minimal change disease) is Immunosuppressive therapy with corticosteroid medications such as prednisone. Most children will get improvements remarkably within two weeks, and nearly all within six to eight weeks. The nephrologist may make dietary recommendations to maintain appropriate protein, sodium, and fluid intake. Minimal change disease fully recovers, without damage to the kidneys, with proper and timely treatment in most children. However some children experience frequent relapse of symptoms through adolesce and sometimes into adulthood. Children taking corticosteroid medications have a somewhat increased risk for infections during the course of therapy and some other side effects which may influence their growth.
